Charlevoix County COA Beaver Island wellness check

Hello Friends,
To all Beaver Island residents participating in the Charlevoix County Commission on Aging Wellness Check Program:
New forms for the Wellness Check Program are now available for pickup at the COA office or at the Beaver Island Community Center (BIC Center) when I am there. Please note the COA mainland office officials have informed me that this form needs to be completed each year to ensure we have the most up-to-date information.
COA officials have said, the completed forms will be coordinated with the Charlevoix County Sheriff's Office on the mainland.
