We are over 65 but will not be on the island that day. Where can we find the survey or is it only a survey for that day?

Unable to attend the dinner, but interested in completing a survey? No problem! A survey can be mailed to your attention by calling the Rural Health Center or just dropping by* during normal office hours (M-F 8:30am - 5:00pm) to pick one up. Completed surveys need to be returned to the Health Center on or before Friday June 17, 2022.
